GP Fertility Referral Checklist
This checklist is designed to support an initial fertility assessment and help make referrals more informative for patients and their treating team. It summarises investigations that may be considered for the female partner, male partner and couple.
Not every investigation is needed for every person. Please tailor testing to the clinical context and include available results with the referral where possible. Referral can be made before investigations are complete, particularly when there are risk factors such as age over 35, irregular or absent cycles, recurrent miscarriage, known male factor or a need for fertility preservation.
